Reach for Life Home Community Based Care was established in 2001 by volunteers from villages and municipalities of Limpopo province. Their goal is to prevent the rapid spread of infectious diseases such as HIV/AIDS, TB, STI, and to support orphans and vulnerable children in their province. The organization has partnered with SOS Children’s Village, South African National Tuberculosis Association (SANTA), African Medical Research Foundation (AMREF) and other organizations from nearby villages. Reach for Life Home Community Based Care currently has DOT supporters who supervise TB patients in Limpopo on a daily basis.
REACH for Life Community Based Care
Identification of TB/HIV patients in the community through symptomatic screening; Conduct home visits to assists and directly observe the TB patients already receiving DOT support including the new smear positive pulmonary TB cases; Support patients to access the TB treatment; Trace TB contacts at home and provide referrals to clinics; Assist in the collection of sputum ; Conduct ACSM events and door to door to motivate TB patients to go for HCT; Provide care and support to patients.
